Person Centered Planning for Supported Individuals

Engage in discussions around what it means to develop a Person Centered Plan and walk through utilizing the suite of tools, including the Personal Focus Worksheet, Individual Agenda, Individual Plan, Charting the LifeCourse. This is a hands-on session. Attendees must bring their own device to participate, preferably a laptop.
